Correct . 2nd phrase modifies Olga , 3rd phrase modifies her father . SV is correct.

B. Grand Duchess Olga Nikolaevna, a nurse of wounded soldiers during World War I, the eldest daughter of Tsar Nicholas II, who was the last autocratic ruler of Russia.
No verb for the subject Olga

C. The eldest daughter of Tsar Nicholas II, Grand Duchess Olga Nikolaevna, the last autocratic ruler of Russia, nursed wounded soldiers during World War I.
3rd modifier should modify Tsar .

D. During World War I, the eldest daughter of Tsar Nicholas II, the last autocratic ruler of Russia, Grand Duchess Olga Nikolaevna, nursed wounded soldiers.
highlighted modifier is ambiguous

E. Nursing wounded soldiers during World War I, Grand Duchess Olga Nikolaevna, the eldest daughter of the last autocratic ruler of Russia, Tsar Nicholas II.

What is wrong with D? I am of the opinion that 'the last autocratic ruler...' acts as the modifier to Tsar Nicholas.
Experts please help!

The Modifier "the last autocratic ruler"is coming between "Tsar Nicholas" and "the duchess Ogla Nikolaevna" and whic is ambiguous; it may modify the Tas Nicholas or Ogla Nikolaevna.

I suppose only A is best others all are wrong modifiers.

The modifier "the last autocratic ruler of Russia" is nested within the modifier "the eldest daughter of Tsar Nicholas II, the last autocratic ruler of Russia".

During World War I, [the eldest daughter of Tsar Nicholas II, (the last autocratic ruler of Russia,)] Grand Duchess Olga Nikolaevna, nursed wounded soldiers.

Grammatically this is not a problem. However the comma after "Nikolaevna" spoils the sentence. There is no need for a comma between the main subject (Grand Duchess Olga Nikolaevna) and the main verb (nursed ).
