Half of the Polar glaciers have melted and this melting process has caused increase in Sea level.
So "resulting" is good modifying previous clause.
As per original sentence subject(glaciers) have performed 2 actions, which is wrong. Since glacier itself can't result into rise in Sea level.

C is Correct; as "comma-ing" type is known for cause and effect.
