I would go with C

The problem with A - It seems that Helen's mother had two fatal attacks.

Hmmm, C sounds correct.

If she suffered a 1st fatal attack, she would not be "recovering" in the hospital

TeCHM / Could you explain a bit more in detail? I don't understand the logic in (C). What's different between "second fatal" and "second and fatal" ?

2nd fatal = she suffered a 1st fatal attack

If she suffered a 1st fatal attack, as A suggests, she would be dead already and would not be 'recovering' in the hospital.

C conveys the message across. While she was at the hospital, she suffered another attack and died.

C as well. If the first is the fatal attack, she was unable to recover.

We need 'when' so remove B and D
Take E out as well as the sentence is awkward
A is wrong as it suggests the first attack is fatal
C is the best choice
