The right answer here should be C. There is a purely grammatical way to solve this, but on a time-sensitive exam like the GMAT, you should always be looking for clues to help you arrive at the right answer quickly. In doing so, you must consider the intended meaning of a sentence.

Let's look at "second fatal attack". An attack (or anything else) is considered fatal if it kills the person/entity it is fatal to. Hence, it is NOT possible for Helen's mother to have a second fatal attack, because the first one would have killed her. She merely suffered a second attack, one that also happened to be fatal. Immediately, this allows us to eliminate A, B, D, E, leaving C as the right answer.

There cannot be 2 fatal attacks. Any fatal attack will kill the concerned person. So "Second fatal attack" makes no sense at all.

I often come across some SC questions where I do not know the exact meaning of some words (in this case, I knew fatal is bad.. but not enough that it is used for something that causes death) hence I answered A.

Is there any general list of words whose meanings should be known or do I need to spend extra time while solving questions to figure out that GMAT is tricking me with these kinds of words?

I'm sure no such list exists, though. :(

i just learnt something very crucial. that if something is fatal it will definately kill you. therefore someone cannot suffer a fatal attack twice.
