The reference to Virgin mary should be Who and not Whom. WHenever there is a confusion between who and whom, just remove the words in between and read. In this example, remaove they say. You will find that it becomes virgin Mary, who has continued to appear.

The answer is D. Though I cannot explain fully why D is correct, I feel that D is correct. I can only say that have had will not come in such a case.

A is the best of the choices. claimed to have had is the correct thing to say. cf: He claims to see visions etc. -- present tense
He claimed to have seen visions etc. -- -- past tense

He claims to have visions etc -- present tense
He claimed to have had visions etc -- -- past tense

If u replace "see' by 'have' and "seen" by "had" things will be clear. That is so far as it goes.

The point here however is the use of who or whom. The pronoun for visions of Virgin Mary should be which and not who/whom. Surprisingly none of the options has 'which' , which is the correct pronoun to be used here.
