D for subjunctive mood.
"demand to V" is corret.
"demand that S + bare infinitive" is correct.

This is testing "Past form of Subjunctive Mood".

....consumers who demanded that it.... is the crux of the problem

This might be one reason why people demanded the old formula be returned:

"According to popular belief, the coca leaf extract cocaine was once added to Coca-Cola, per se. Because cocaine is naturally present in untreated coca leaves, small amounts of cocaine were also present in the beverage. Today's Coca-Cola uses "spent" coca leaves, those that have been through a cocaine extraction process, to flavor the beverage. Regrets, wishes, demands are expressed with subj mood:

I demand that he be
I require that he go
etc.

But require can be also constructed as following
I require him to go

So why cant we say
I demand it to go??

(D) - pronoun reference, who clarifies that the customers are demanding - and 'it' would refer to the company. 'that it bring back' is also correct usage.
