So here the comparison is between the design of the hospital in theory and in practice. So here verb "is" shows that the sentence is the present tense so why cant we use do??

We can not use "DO" because the "hospitals" themselves are not designing.

The Hospitals are designed by someone. So, we have to say:

In theory, a hospital is designed as per disease pathways, but, in reality, only some hospitals such as Arvind Eye Hospital are/were/will be [designed].
