IMO, I do not agree with B. My pick is D.
First of all, "require X to do Y" is subjunctive form, so I eliminated A, C, E. Only B and D remain. Let analyze each option:

Management is considering a policy requiring employees provide doctors' notes after taking sick leave so as to prove fitness to return to work.

a-provide doctors' notes after taking sick leave so as to
Wrong. Subjunctive form: require X to do Y

b-to provide doctors' notes after taking sick leave so as to
Wrong.
- The structure is like: "Management requires employees to provide doctor's notes ...so as to prove fitness..." It's quite not clear, who will prove fitness? management or employees?.
- From my experience, "so as to" is not preferable to "so that" or "in order to" in GMAT (this is not a rule, just my experiences).

c-provide doctors' notes after taking sick leave in order that they
Wrong. Subjunctive form: require X to do Y

d-to provide doctors' notes after taking sick leave so that the employees can
Correct IMO. The present of subject "the employees" after "so that" is very clear.

e-provide doctors' notes after taking sick leave and
Wrong. Subjunctive form: require X to do Y

Read the Original Sentence Carefully, Looking for Errors:

Usage of "so as to" can be considered wordy and awkward. Remember to go for the most concise choice.
SO THAT is better usage

Eliminate Wrong Answer Choices:

Choices (A), (C), and (E): In this case, "requiring employees" must be followed by the infinitive "to provide." Eliminate choices (A), (C), and (E).

Choice (B) must be eliminated, since the use of "so as to" makes it sound as if employees take sick leave in order to prove fitness.

Answer Choice (D) is the only choice that uses the correct verb and also makes it clear that the employees must prove fitness. This is the correct answer.

Command subjunctive construction is used in this question thus we will have to take care and identify out of two basic command subjunctive format which we can use here.

Command subjunctive key words: advise, allow, forbid, persuade, and want (ONLY takes infinitive form) and ask, prefer, order, beg, intend, order, require and urge (May take any one of the formats with infinitive or with That clause) Refer page 114, MGMAT SC for more detail.

Management is considering a policy requiring employees provide doctors' notes after taking sick leave so as to prove fitness to return to work.
A. provide doctors' notes after taking sick leave so as to -- command subjunctive format is missing.
B. to provide doctors' notes after taking sick leave so as to -- so as to <confusion key word> -- who is the subject of action described after so as to management or employees. therefore, incorrect.
C. provide doctors' notes after taking sick leave in order that they --same as A.
D. to provide doctors' notes after taking sick leave so that the employees can -- perfect as per the format.
E. provide doctors' notes after taking sick leave and -- same as A.

There are two errors in the original sentence
1. provide should be changed to to provide
2. Management is considering a policy ........so as to prove fitness to return to work - This structure gives the feeling that the management wants to prove fitness. Whereas, its the employees who have to prove fitness. Therefore, "we need so that the employees can " to make the meaning clear

A.provide doctors' notes after taking sick leave so as to - Incorrect. two errors as discussed above
B. to provide doctors' notes after taking sick leave so as to - Incorrect. meaning error as discussed in point 2
C. provide doctors' notes after taking sick leave in order that they - Incorrect. to provide is required. they is ambiguous.
D. to provide doctors' notes after taking sick leave so that the employees can - Correct.
E. provide doctors' notes after taking sick leave and - Incorrect.

I think ans is - D

Require X to and require to are correct idioms, so A, C, E are out.

Left are B and D.

Option B changes the meaning. It means that the management has to prove fitness to return to work.

Option D has the correct meaning. :)
