It took me close to 6 months and the preparation was intermittent, I sat for 2 months then I didn't touch the book for another 2/3 months and back with the books again. I would say 3 months with 15 to 20 hours each week would be suffice for a 50 to 100 point score improvement.

First GMATPrep, absolutely zero preparation: ~720 (July 2011)
Final GMATPrep: 760 (June 2012)

3. Native english speaker
Yes

4. Study material used before first exam and between the following tests
OG 12 - used the explanations in this guide to correct my mistakes
GMATPrep software - used this software to simulate the real test environment

5. Time frame
Although my first GMATPrep was in July 2011, I only studied in May and June of 2012.
