The gap in your candidacy is clearly the Ds and Fs, and the C. What is the overall cumulative GPA? You are going to have to really tell your story about this well. Obviously you got a bulge bracket job out of school despite those grades- what was your story to them? You need to use this as an opportunity to show a path, how you have matured, how you persevere, turn this "weakness" into a strength. If there was something "personal" behind those grades, I would share. But, above all else, make your self look stable. The grades give cause for concern. I would emphasize your performance reviews of the past three years as evidence of your ability to excel for an extended period of time.
As for the extracurriculars, emphasize your leadership roles within them and the initiatives you took, and any innovations. You want to emphasize your leadership qualities here, and via your work experiences.
Lastly, why an MBA? Your profile reads that you want to transition form IB to PE? Why do you need an MBA to do so? You need to answer this for the AdCom.
Also, brainstorm what differentiates you from the other IB candidates applying to these schools. How will you uniquely add to the classroom?
