Let's break the sentence using C as the option.

Now, I believe you know that is used as a restrictive modifier and it is referring to Nineteen and not the islands. So, if I remove the modifier, my sentence would become

Polynesia is an archipelago with at least thousand islands, nineteen.

Now, this nineteen is not making a complete sentence. Hence, C is incorrect.

Whereas in option B, we have used "which". This is refering back to the noun before the comma and it is implying the meaning that those islands are officially recognised.

Polynesia is an archipelago with at least thousand islands, among whom nineteen have official recognition.
