Ask yourself, what implemented properly ??? is it the decision or the concept of care system. That is the key of this question.

it

not they (Singular antecedent).
Secondly,

decisions are

is correct. Were makes it past.

Between B and E, but chose B in the end because it’s less passive - Accounts make decisions vs decisions are made by accountants. Why is E correct?

Though the concept of managed care may evoke for some people images of a health care system in which not doctors but rather accountants make the decisions, when implemented properly they could result in better overall health care at a substantially lower cost than that of non-managed systems.

I was left with B and E. In the end I chose E. Doesn't B incorrectly imply that accountants also make doctors? I know it sounds weird but hence its wrong according to my reasoning. The structure in B is "accountants make X rather Y".

In contrast to that in E we use the correct structure "decisions are made by X rather than Y". This conveys the correct meaning.

I would appreciate feedback on my reasoning :-)

The difference in answer choices which I noted was usage of "it" and "they" in when clause after comma

Sentence is talking about concept so "they" is incorrect so A and C are out

D is wrong due to usage of "were made".no past is required here

Bw B and E
Sentence stresses on what happens in concept wrt meaning "decisions" at start makes more sense
And sound more appropriate when compared with accountants..

Though jag of passive makes E less selective but meaning wise it is surely better than B


Also could and can
Could refers to less possibility whereas can refers to more possibility.we can go through this as well

A and C are out since they both have the same SV disagreement. They both use ‘they’ to refer to ‘the concept’. D improperly uses the past tense so it is out. B is correct but a little awkward. E is a passive sentence but it is still better than B. B should be selected.
