The best answer is option C.

A. to work in the conjunction of a doctor who ... the is not necessary. the right idiom is in conjunction with not in conjuction of
B. to work in conjunction with a doctor that ... that refers to things not human beings. Hence incorrect.
C. to work in conjunction with a doctor who .... correct. the right idiom is used and who correctly refers to a doctor who is a person.
D. to work in conjunction of a doctor that ... in conjunction of is unidiomatic. The correct idiom is in conjunction with. The use of that is not appropriate as already stated above.
E. to work at the conjunction of a doctor who ...at the is inappropriate. The right idiom is in conjunction with.

MEANING
Athletes need to work IN CONJUNCTION with a DR WHO understands this and can design x;
When referring to a person, we need "WHO" not "THAT".

Doctor is living specie, so use who instead of that, which is used for non-livings.
