IMO the best choice is E.
All other including A is using their as a pronoun for everyone.
However D is completely different in meaning as it is talking about terrible negligence!!

OFFICIAL ANSWER:

E. The pronoun everyone is singular. (If somone doesn't want to trust me on this, one can use this pronoun in a simple sentence. "Everyone is happy that you are applying to bschool" versus "Everyone are happy that you are applying to bschool.") Answer choices A, B, and C can now be quickly eliminated. Answer choice D changes the meaning of this sentence too much. One could reasonably argue that the speaker of the sentence did not intend to convey that strong of an opinion on the accident.

Option E is correct.
Simply apply the subject pronoun agreement.Since everyone is always singular , singular pronoun should be used.

Ideally, option E should have 'his or her' to refer back to everyone. Anyhow, E is still the best option.

E. to show that everyone must be held responsible for his actions. (Correct)

I think in option E GMAT would rather use "one's actions" instead of a gender specific pronoun.
