Option E is correct.
Simply apply the subject pronoun agreement.Since everyone is always singular , singular pronoun should be used.

Ideally, option E should have 'his or her' to refer back to everyone. Anyhow, E is still the best option.

The Navy used the nuclear submarine accident off the coast of Hawaii to show that everyone must be held responsible for their actions.

A. to show that everyone must be held responsible for their actions. (Everyone is Singular)

B. to demonstrate their belief that everyone must be held responsible for their actions. (Same as A)

C. to demonstrate its belief that everyone must be held responsible for their actions. (Same as A)

D. to signal how no one should be able to get away with committing acts of terrible negligence. (Changes the meaning)

E. to show that everyone must be held responsible for his actions. (Correct)

I think in option E GMAT would rather use "one's actions" instead of a gender specific pronoun.
