It should be D,

A and B are out because of not using 'that'

C and E are out cause 'spend/ing' incorrectly refers to the insurance plans and not the patients.

patients, spending.... in C

Spending here correctly modifies patients. The removal of the comma and/or use of and makes the word refer back to insurance plans in C and E.

What is the OA, I will feel like a dork if OA is anything else after this explanation!!! :-D

Part of learning is figuring out why we're wrong and not doing it again! Don't feel like a dork for getting something. Learn from it and don't worry about what anyone else thinks.
