My vote is for A. Within this option harm was intended even though it did not result in the same. For all other options the harm could not have been thought of/anticipated earlier.

Hi!

HINT: The OA claims that harm could have been foreseen in one case. So, dig deeply.
I'll post an explanation later.

P.S. I made a mistake with this question and just want to see that I'm not alone in the trouble :)

I agree with OA D,
She must have visualized that if she diverted her attention, or "not look at cat", it may get in danger. Being aware of the consequences, she decided to watch TV. Hence, an incorrect action, as per the thought stimulus

Yes, after seeing the OA the stuff becomes easy, but without both the hint and OA the question looks like a LSAT trap rather than GMAT question.

Hm, I narrowed it down to D and E and went with E. I'm having trouble seeing the difference here. In both cases, the person was doing something that had no intended negative consequence, but ended in hurting someone (the cat in D, the homeless man in E). In neither case did the action of the person DIRECTLY cause harm - in D, she opened the window and cat fell out of it; in E, she gave the homeless man soup and he choked while eating it.

I don't see why D works and E doesn't...
