It could be something they require someone else to do. The rest of the sentence talks about medical lawsuits so we know that we are talking about giving protection to doctors and doctors purchasing this coverage. So the sentence has to be framed to reflect that.
(E) Doctors require $250k in malpractice coverage to be protected ... Doctors are the ones who require this coverage. Why do they require it? to be protected - so that they are protected - passive
This all makes sense now.
