I wont post what I was thinking since it will come across as CBS bashing and I think its a great school and am not knocking the school just their admissions reputation.

You have to realize that certain backgrounds get admitted almost no matter what. If you got a high GPA from a certain set of schools, worked for a certain company, and did OK on the GMAT you are getting into the top schools. You could write your essays in crayon and still get in...Schools know this and they know what applicants they have at best a 5% chance of getting to attend. Most of these people made or will make so much money that a fullride means nothing to them.

What is the methodology used by PR to get this? Is it acceptance rate, i.e. admitted (offered or final class size?) / total applicants?

I think that schools also try to increase yield by accepting those students who have a more demonstrated interest, not just qualifications, into their programs. Although on their part, I'm sure this is easier said than done. I think it's reasonable to suppose that for an uber-qualified candidate, they will put much more effort and homework into a H/S/W application than to their "back-ups" of lower M7. Therefore, if CBS Adcom comes across an uber-qualified applicant with so-so enthusiasm, or specific reasons for wanting CBS, they could treat this as a yield increasing ding.

It really depends. If someone has an outstanding background, high GPA and GMAT, great ECs and is typical of the type of student that gets admitted to H/S/W, then I can see it happening.

However, it's probably much more common with the lower ranked schools where the difference in ranking is much more stark.
