1) Brief description of your full-time work experience. What do you do?
Past year: Intern at Morgan Stanley Wealth Management. Research various securities, create portfolio reports, manage junior interns. Btwn 20-40 hrs/week.
Upcoming year: Job hunting for equity research position.

2) Your GMAT.
780 (99%): Q49 (79%) V51 (99%) I8 (92%)

3) College info: The name of the college, your GPA or grade average, your major, year of graduation.
Johns Hopkins University. August 2013. B.Sc. in Business. 3.93 GPA. With honors.
Ner Israel Rabbinical College. August 2014. B. in Talmudic Law. 3.88 GPA

4) Significant college and post-college extra-curricular activities or community service, especially leadership experience.
Consult for online wealth advisory company, most running portfolio models. (2014)
Founded and ran intramural floor hockey league at Ner Israel. (2011-2013)
Teacher's assistant at former high school (2013-2014)

5) Important certifications like CFA, CPA, FSA, or CA.

6) Your target programs. Please note that my experience is with the top MBA programs (FT, PT, EMBA, Sloan Fellows) around the world. If the programs you are targeting are not listed here, I may not respond due to lack of time.
Harvard, Columbia, Wharton

7) When you plan to apply? Please specify round and application cycle, for example R1 2013-14 or R3 2012-13.

Your numbers are certainly impressive and will ensure your application receives a serious review by your target schools.

The key in your situation will be to show (a) how, with your relatively short experience, you will “carry your weight” as a contributor in classes and on projects with classmates who are also stellar performers and deeper experience, and (b) why you need the MBA at this early point in your career.

I believe that if you make a compelling case for (b) above and present/integrate your differentiating factors (e.g., expertise in Talmudic Law) effectively, you will be well positioned for the top programs you are interested in.
