I also picked E.
A) Promotions, retirements, deaths, and other actions.... although parallel, its wrong, death cannot be including int he list
B) Along with death... to me sounds similar to A
C) same mistake as above plus 'to be reported'
D) same issue of including obituaries in actions taken by the board, plus its awkward.

I would go with E

The reader is bound to get confused reading A. The other options are either wrong or confusing. E states it correctly by putting deaths in different category.
