AndrewN I think the error in D is that the construction expresses an ambiguous meaning. One way of understanding it is the way you explain but I consider that another way is how TheUltimateWinner does (The list would be 1. food high in whole-grain fiber, 2. vegetables and 3. lean proteins).

Additionally, I eliminated D because it changes the meaning of the original sentence:
