'Swum' is the only past particple that can accompany the past perfect 'had'. 'Swam' is a plain past tense and cannot pair with the 'had' as in A and E.
The simple past 'swam' in B and C doest not fit in becuase the act had already been completed when I jumped in. D is the correct expression
