A seems to be the best option.

Parallelism

Deborah Sampson,..., joined.. at the age of 22, was injured... and was discharged....

Option B - 22, was injured... , while being discharged - illogical
Option C - 22, injured... and was discharged, being
Option D would have been correct if it was just "22, injured ... and discharged"
Option E - 22, having been injured....... and discharged, being

You need the perfect past as the time when she became ill precedes the time when she was discharged.

The most important thing here is parallelism.

joined.. at the age of 22, was injured... and was discharged....

It is also important for Verb Tense. She had to be ill first before being discharged.

Hence A

my pick is (A)

A) 22, was injured three times, and was discharged in 1783 because she had become - Correct - Parallelism and correct tense. She had become ill before she was discharged

B) 22, was injured three times, while being discharged in 1783 because she had become

C) 22 and was injured three times, and discharged in 1783 being

D) 22, injured three times, and was discharged in 1783 because she was

E) 22, having been injured three times and discharged in 1783, being
