The TOEFL requirements vary by school & your best bet is to check with your target schools directly to find out whether or not you need to take the test. If your undergrad was taught in English most schools will waive the test. It's usually based more on that than on visas. But like I said, double check with each school to be sure since each one has its own unique requirements & guidelines. I'm guessing based on your post that it would be easy for you to get a good score so hopefully that won't feel like a big deal if you do need to take it.
In terms of getting a 2nd MBA, you'll want to check directly with schools there as well. Many schools will allow you to apply if you already have an MBA, but there are a number of schools who will not accept your application (for example, Haas, Tuck, Ross). Some schools explicitly state on their website that they do or don't accept applicants who have an existing MBA, while other schools don't give any guidelines (in which case you'll want to check with them directly). Even among schools that do let you apply if you already have an MBA, you'll need to be ready to make a strong argument for why you need an additional one in order to fulfill your career goals. Although they recognize that Indian MBAs for example are quite different from traditional MBAs, it does still make your job a bit harder when it comes to convincing them to let you in.
