Option B seems more appropriate to me!

Smoking is unhealthy to such a large degree that it could significantly shorten one's life.

A. Smoking is unhealthy to such a large degree that it could significantly shorten one's life - "to such a large degree" is a little bit wordy construction

B. Smoking is so unhealthy that it could significantly shorten one's life - correct choice

C. Smoking is unhealthy to a large enough degree as to significantly shorten one's life - "to a large enough degree" is a worse way to say "so unhealthy"; plus I've read in some material (Manhattan?) that "as to" is rarely in the correct option in real GMAT questions

D. So not healthy is smoking that it could shorten one's life to a significant extent - "to a significant extent" vs "significantly" - style issue: adverb always wins the phrase

E. Smoking is so unhealthy that it could shorten one's life - concise but miss the emphasis ("significantly shorten")
