In this quetion i m confused between B and D ...How to go about it ....anybody who can help......

which is clearly wrong in choice A, because usage of which is ambigious.

here you need pronoun ("that" as opposed to conjunction "and") to link two independent clauses "Keralans are by far Indiaâ€™s most literate citizens" & "There are more than forty newspapers published in the cities of Kerala"

Remove the modifier in red and you are left with:
There are more than forty newspapers published in the cities of Kerala and that number reflects the fact that Keralans are by far Indiaâ€™s most

"and" is necessary here because you are linking 2 independent clauses; a conjunction is compulsory. Just remove the "and" and the sentence would not make sense anymore. It would sound like Kerala was a number that reflects... without the "and", the second part would become a restrictive clause.
