D. to eliminate hazards from the residential environment and, combined with balance training, to encourage an increase of bone-density through diet and weight-bearing exercise, before bone-loss develops. --> Correct, Maintains parallelism and sequence of mentioned events.

I'd go with D

First split : to vs if

The best strategy is to -> correct
The best strategy is if -> wrong

Among A, B and D
A -> eliminate hazards from environment before bone loss is senseless. Combined is used used as a modifier, which modifies closest noun I.e bone loss which is again wrong.

B -> incorrect
to eliminate hazards and a combination of balance training gives wrong meaning.

D is crisp and maintains parallelism with meaning.
