The nurse told me that the doctor’s office closes at 5:00 P.M.

Told is in the past tense and closes is in the present continuous.
Since office closing is an everyday activity it should be in the present continuous form.

(A) The nurse told me that the doctor’s office closes at 5:00 P.M.
Correct usage of tenses.

(B) The nurse told me that the doctor’s office closed at 5:00 P.M.
Incorrect usage of tenses.

(C) The nurse had told me that the doctor’s office had closed at 5:00 P.M.
Incorrect usage of tenses.

(D) The nurse told me that the doctor’s office had closed at 5:00 P.M.
Incorrect usage of tenses.

(E) The nurse told me that the doctor’s office would have to close at 5:00 P.M.
Incorrect usage of tenses.

SajjadAhmad :- I see that even option B could be correct.

Why can't the nurse say that the doctor's office closed at 5:00 PM. (May be she met me later after the office was already closed).

All options but C could be correct, depending on the sequence of events.

Official Explanation

The sentence is correct as written. The only question is whether the present tense of the verb is correct. In this case, the sentence uses the present tense, “closes,” to assert that this is the usual practice of the doctor. Each alternative answer choice changes the meaning of the sentence.

Thus, each alternative choice is incorrect, because it makes a gratuitous change from the original and in so doing, changes the intent of the original sentence.

The correct answer is (A).
