This is how i solved

Let the no of men be x and women = 4 more than men so X + 4

We know X + X+4 = 16
Solve for Xie 2x = 12 x =6

So men in the team is X ie 6 and women = 16 -6 = 10

Ratio of men to women = 6 / 10 ans D

This way 3/5 doesn't follow any of your mentioned conditions, still if given in options it would have been the right answer.
Ratios cant be trusted to follow the equations of the actual numbers of Men and Women they represent

We can let the number of women on the team = w and the number of men = m. Since there are 4 more men than women and a total of 16 players, we can create the following two equations:

1) w = m + 4

2) m + w = 16

Since m + 4 = w, we can substitute m + 4 for w in the equation m + w = 16 and we have:

m + m + 4 = 16

2m = 12

m = 6

Since there are 6 men, there are 16 - 6 = 10 women, and thus the ratio of men to women is 6/10.

W = M+4
Add M on both sides=> M+W = 2M+4
M+W is given to be 16 => 16 = 2M+4=>M=6
Therefore W = 10
M/W = 6/10
