Well, each person follows a different path, so it's hard to say what is normal.
What you will need to get in will also depend a little on the school, but usually:
- transcripts and diplomas from undergrad and above (as well as certified translations, since your transcripts and diplomas will not be in Englçish);
- a high GPA in those transcripts;
- resume (usually research experience is important, teaching experience a little less, professional experience a lot less);
- letters of recommendations (usually 3 of them, preferenc given to recommenders who are researchers);
- TOEFL (specially for US) or IELTS (specially for Europe) to show proficiency in English, since you are a foreign applicant;
- GMAT or GRE, with very good scores (PhD is usually more competitive than MBA);
- your statement of purpose;
- payment of application fee (around $ 100 each);
- online application form.
Some schools will have additional requirements. It seems schools in Europe often ask for a research proposal too, but that's not common in the US.
You can apply to PhD programs without a master's, but many applicants do have a master's degree and so their profiles are potentially stronger.
