This happens to me too! I am trying the following
1) Before I start any test/question bank I remind myself to be very careful and pay very close attention.
2) Spend a couple of seconds after a question to check my calculations
3) And like BB said, remembering to slow down(esp during the calculations)

will do, ugh, what restaurants are open on christmas for breakfast need to get my fill of bfast before killing this gmat study first
