I am still at company X
2 months ago, my boss left company X.
He just started at company Y.

On the recommender information, is my boss considered a direct supervisor at

"current employer"
or
"previous employer"
?
Ie., is the current/previous employer question from my perspective or the recommender's perspective?

Also, do I put down my boss' address at his new work place or his old work place?

I'm in the same boat. I wouldn't call this person a direct supervisor but a past supervisor at the current employer. Obviously enter his current contact information.

In the application the "current employer"/"previous employer" is referring to you, not the recommender. So as staind said he would be a previous direct supervisor at your current employer. Usually apps have space to explain why you didn't choose your current direct supervisor. The fact that he left 2 months ago is a very good reason not to use your current supervisor so I wouldn't worry about it.

Definitely current employer if you are still at the firm. It probably isn't even that important, because the exact nature of the relationship and timeline should be disclosed in the first paragraph of the letter anyway.
