What concern scientists the most about global warming are the risks that the polar ice caps will melt, the seas will grow too warm to sustain marine life, and that violent weather patterns may result.

What is important here is the plural word - 'concern' hence the verb 'are' is correct here.

The next thing we should be looking for if the list is parallel and this parallel structure can be best established per below:

- are the risks:

----that the polar ice caps will melt
----that the seas will grow too warm to sustain marine life, and
----that violent weather patterns may result

Hence - Option C is Correct

1. Would the original st be correct if the last element after and did not have 'that'- in which case the 'that' after risks could have referred to all the 3 elements being joined? and hence II maintained?

2. Do all elements being joined need to have a preposition attached? Either one single one in the beginning that applies to all, or each individually?

Yes, if that last "that" were not present, the original version would be correct.

Here 'what' is placeholder which follows by plural concern because 'risks' is plural but now "What concern scientists the most about global warming" is the subject,which is a clause and it should be followed by singular 'IS'

To answer your query, since "what" refers to the plural noun "risks", it too is plural, meaning the subject of this sentence "What concern scientists..." is plural and requires the plural verb "are".
