It cannot refer to an action. So, choices A, B, and E are out.

Among C and D, D wins because of the usage of "not only" with "but also"

D. not only to always add acid to water but also to do so by stirring slowly, as doing so

Correct idiom 'Not only, but also'.
Pronoun 'it' is confusing in other options, 'Doing so' over 'doing it'.
'Doing so' clearly refers to the sequence of preceding actions.

Hence, D.
