Which (and probably that) can actually modify two things:

1) The noun just preceding it, OR
2) The noun + prepositional phrase just preceding it.

You have to use the context and grammar of the sentence to figure out what "which" is modifying.

eg The house by the lake, which was owned by Jack, has been sold. => Here which is modifying house by the lake => house, and not lake, though lake is the closest noun.
