B WINS. The verb should be singular because the subject is "One"

IMO-B
"One" of every two business is singular & hence it should be "fails" and not "fail".

B question though... when would it be "has failed"? Only if the sentence started with something like "Since ..... one of every two has failed" ?

So it would be like this: "one out of 2 fail/fails" One out of 2 FAILS - that's (B). So pick (B) and move on. Hope that helps.
