HBS self reported transcript form - help! please [#permalink]

Hi all,
I've been trying to download the self-reported transcript form from HBS for a few days now with no luck. Apparently there is a mess up with my browser or something.
Would anyone be kind enough to send me such form or at list tell me what columns to include? I have all the data in an Excel spreadsheet and my app. is otherwise ready, but I'm still struggling with this issue.

Isn't this template based on Stanford GSB form? Can it raise any red flags for HBS?
