Given your profile (solid middle-of-the-road candidate for the top schools), you're choosing a good range of schools.
HBS and Wharton are stretches but you probably already know that. Go for it, as long as you're keeping realistic expectations and applying to other schools as well.
Kellogg and Booth are slight stretches (there's enough folks of a similar caliber at these schools, but plenty who didn't get in either). But still worth applying to.
Yale, Ross, and Fuqua (and others in this range like NYU, UCLA, Cornell, Darden) are your sweet spots. I think you have a reasonable shot at these schools and where you should focus most of your efforts on. Choose 3-4 from this list. And Duke is known to have a great healthcare program anyhow, so it should be one of your top choices.
