returned back,
recurring over and over
These are redundant structures.
(B) is the best choice.

I feel this is a very good example of redunducy, verbosity, short and sweet answers and crisp tone.
Well between B and E, I choose B as my FA.

I believe the correct usage of continue is
- continue to do
- continue doing

This post received KUDOS
'return back' -> 'back' is redundant. A and C are out.
In (D), 'continued on playing' is awkward. The 'on' is not required.
Between (B) and (E), choice B is a better choice.
In (E), 'hoping that the return' will result in an awkward sentence.
"in spite of" and "despite" both means the same thing. (Read this link for more information on 'in spite of' and 'despite' http://www.learnenglish.org.uk/grammar/archive/contrast01.html)
B for me!

What is the significance of the tense differences of to play and playing?

THis was an easy one. B for me.
When I read the question itself I saw 2 very obvious mistakes.
1. "continued on playing" - to make this right there are 2 options. either change it to "conitued to play" or to "continued playing".
2. "to return back" - this is redundant. return and back mean the same thing.
based on 1. I ruled out A and D immediately.
based on 2, I ruled out C.
Now I am left with B and E. In E "hoping that the return to his position as a regular starter in the game he loved. " This sounded wrong to me. So I chose B.

lol. not a tense difference.
A. recurred over and over..redundant
B. correct
C. return back..redundant
D. continued on...redundant
E. incomplete sentence.
