E is the most concise and uses not only/but also idiomatically and //y.

A & B are not //
C & D use passive form in the second part of the clause.

Only E is concise and avoid //ism issues

saurya, This should be of the form 'not only by X but also by Y' where X & Y should be parallel but in 'A' these are not parallel becos X is 'the foods patients eat' and Y is 'when the patients eat them'

THe main subject in x in option A is food
and the main subject in y is patients

where as in E

both x and y are having patients as their main subjects...
