I go for E.
the surface broke apart as a result of repeated impacts , after which the fragments rejoined through

"after which' modifies "impacts", so it needs to be close to "impacts"

A is passive. In B and "having rejoined" is wrong, and C is ugly and wrong. Is E

repeated impacts broke or the surface broke apart?

It is at one time the surface broke apart

D) having rejoined is incorrect in D.
