for me ensure ..must - redundant
So between c, d & E
Thougth there is a flaw in 'C' i would go for 'C' . are ...to ensure - parallel

Eliminate answers with "must". It is a nonsense to say " to ensure that someone must be..." C sounds good but we must get rid of it because, using the present tense, an indipendent panel doesn't not evaluate, but evaluateS. D is incorrent because it uses the future and the conditional. E is parallel and correct.

My choice is E.
The federal rules ..... were established to ensure that patients
would be warned.....
A. using past form so, "would" is preferable.
B. ||ism : that x and that y
C. Also would is || on both the sides of "and"

The federal rules aimed at protecting human subjects of medical experiments were established to ensure that patients must be warned of potential risk and an independent panel would evaluate the experiment before it was conducted.
Here are my observations.
1. WERE establised- Past tense with Plural Noun Federal Rules 2. ENSURE - Meaning of ensure is to make sure. 3. WAS - Past tense with singular noun the experiment.
From answer choices -
a. must be warned of potential risks and an independent panel would - ENSURE....MUST are redundant. So ruled out.
b. must be warned of potential risks and that an independent panel - ENSURE....MUST are redundant. So ruled out.
c. are warned about potential risks and that an independent panel - All the verbs in the sentence are Past tense. ARE is trying to make is present tense. So ruled out.
d. will be warned about potential risks and an independent panel would - All the verbs in the sentence are Past tense. WILL BE is trying to make is present tense. So ruled out.
e. would be warned of potential risks and that an independent panel would - gooes good with the sentence along with the || rule - to ensure that patients would be warned......and that an independent panel would....
