Actually, I ignored A first, in the thinking that ETS does not like "that they be accountable" kind of statements. this type of statement can further be shortened.

D. "accty for providers" changes meaning.
C. "have accountability to" is not idiomatic.

B for me.

health insurers are trying to establish whether the treatments are beneficial. "A" says providers are establishing treatments. so, B or D.

A seems to be right to me.

Dj,
Providers are administering the treatments. Are we missing something. To me A sounds logical and complete. Even the pronoun 'they' fits perfectly well to the Mental Health Providers, 'accountable for' is the right usage of Idiom.

Correct me in case i have mised something.

I also got B here since I thought that intended meaning is that ...health insurers want to establish that providers of health services provide beneficial services.
OA given is A. I think either it is a very awkward sentence or I am unable to understand some basic stuff here.
Again a confusing one!!!

It is A. "accountable for V.ing" is nicely an idiom. You can check here
