I think it's a bit of a reach for you to call ahead assistantship extracurricular. It's really more along lines of an internship.
I think your idea about reporting your function industry is fine. If they don't have your specific breakout for job title that's on them. What else can you do right?
With regards to the current student, if they approach said student, and asked hey do you know so-and-so insert your name, would he say yes or she say yes?
I understand your interest in checking boxes, but don't feel like you have to have done everything and check all the boxes to get admitted.
It's really not about checking boxes about being exceptional and contributing something to the class. So not everybody is going to be covering every base.
It's a little unfortunate that you don't have any other extracurriculars, if that's the case, but if there are offsetting entries don't worry about it.
For the last question, you should always submit all your materials in PDF format to preserve the formatting.
I'm not really sure what you mean by questions on top? oh I think you you mean the essay questions at the top of the document. I've seen some school specifically request that you not do that. But if not, then I see no harm in doing that.
Personally when I get essays from someone without the prompts I get irritated.
