GMAT Q's

Some things I've wondered about:
Difficulty levels of questions probably come from how many people get them right when they appear as "practice" questions on GMATs. What if it is just a bad worded question or wrong question and most get wrong? This will be incorrectly classified as difficult yet it may be a bad/incorrect question. Along with this, if there was an error on a question, how would anyone ever know?
How many questions are there in the GMAT pool? How many people are we competing against that got scores based on cheating? I hope there is a wide variety of scores and I hope questions do get updated at least monthly to combat this.
What is harder to get, a 51V or a 51Q? But what about let's say a 51Q and a 48V? I think 48V is probably still harder.
Along with that, what barrier is the toughest to break? I know it depends but overall with everyone? 700 barrier? 50Q barrier? 45V?
