This is such a very tricky question and I initially get stuck between C & D. However, I choose D because the modifier placed here will emphasize strongly for the exhaustion of Larry in this context, than C does

The first time that Larry walked down the ramp, laden with packages, he tripped and nearly dislocated his shoulder.

(A) Larry walked down the ramp, laden with packages, --> misplaced modifier
(B) Larry walked, laden with packages, down the ramp --> misplaced modifier
(C) Larry walked down the ramp, with laden packages, --> no errors, but it does not strongly emphasize the hardness of Larry in dealing with the laden packages
(D) Larry, laden with packages, walked down the ramp -->the best
(E) Larry, laden with packages, had walked down the ramp, --> past perfect maybe not correct

(A) Larry walked down the ramp, laden with packages, misplaced modifier (ramp laden with packages?)
(B) Larry walked, laden with packages, down the ramp laden... breaks walked down - bad style
(C) Larry walked down the ramp, with laden packages, the same as for A
(D) Larry, laden with packages, walked down the ramp the best
(E) Larry, laden with packages, had walked down the ramp, changed tense of verb

IMHO, (D)

imo , D

"laden with packages" modifies Larry and not ramp.
