Schools - Take a look at career placement stats in Operations and also industry specializations too, do you want to stay in Auto for instance? Schools that come to mind on the more competitive side would be Sloan, Ross (especially for auto industry), and maybe Kellogg's MMM. You should apply to a range of schools though so consider some programs outside the top 10 and check out Tepper, Smeal, Krannert, Fisher, Kelley, Broad. I think they all have pretty strong Ops programs.
GMAT - You've got a great score, I don't see a need to retake.
Timing - If you can I would apply in 2015. Reason being that at 29 now, you're just slightly above the average age for most programs (28) and I think waiting another year will put you at a disadvantage.
One potential area to improve might be in your extracurriculars...it seemed like everything you listed was back in college, which just isn't weighted very heavily.

Thanks for the information. Can you please advise which MBA track is appropriate for me in Smeal and Broad as according to there website they don't offer operations management track for MBA.
