The OA B should mention rehabilitation center. I feel it is a poor quality question. Please correct me if m wrong about the rehab center thing.

B it is.

First of all... It should be " both and " . " Increase in both " is wrong as a change cannot be increased

Between B and E :

B states no. Of admissions to hospital emergency rooms nationwide.

E states no. Of admissions to emergency rooms of nationwide hospitals .. may imply that nationwide is a franchise of hospitals..and the no. Of admissions has increased only in those hospitals named nationwide.

Your distinction is absolutely right. Nationwide hospitals seems to be a franchise of hospitals...whereas hospitals nationwide means hospitals all over the nation.

I believe the underlined part needs to be corrected. "Rehabilitation clinics" is missing in all choices except A
