Will go with

I chose A and E.
I have 2 remarks about the OAs
d). an increase that would amount to roughly five miles per gallon and would represent
Is that correct? It is not the increase that amounts, but the efficiency...
d). that impose stricter limits on medical services and require doctors to see more patients, and spending
if there is no typo, it is clearly not correct. What is the first part for // of ..... AND spending?
without the AND, wd be correct.

If "and" were included in option (D) for the second SC, I would have chosen it.
@Jaynayak If your memory serves you right, could you please confirm the source of this question so I can make the necessary revision(s)? No need to include a flawed SC in our notes/error logs...

Completely disagree with your explanation for D.
require in "[require] spending less time." is not implied as you are suggesting.

Completely disagree with your explanation for D. require in "[require] spending less time." is not implied as you are suggesting.
Then what is implied and what should be the correct sentence????

The explanation to D seems REAAAAALY far fetched. PSD: i hope you dont take offense. i am trying to learn more from you guys.
Require doctors to X and to Y seems better than
Require doctors to X and require Y-ing
