Thanks for the explanation. I had a doubt while I was answering this question. The sentence has two noun phrase modifiers in the beginning.
NP 1- Crucial in the decision to sell the company,
NP-2 a decision which made some 300 employees redundant,

Are two back to back modifiers allowed in standard GMAT English? If so, can you help me with an OG example, which has a similar sentence structure ?

Sorry but in my opinion I disagree with this question, mainly because:

is redundant

the part in between the commas and that at the same time is the subject of the sentence, in general is an incidental phrase. You can eleiminate it without a lack of meaning of the overall structure.

we have a passive voice

Considering that is quite difficult to have such sentence on gmat exam. My opinion

I have doubt in this

Why to use "were" ?

“which” introduces information that isn’t crucial to the meaning of the sentence, known as a non-restrictive or non-essential clause, while “that” introduces a restrictive clause. - For this reason D) is correct choice??????

honestly i am not familiar with this lingo and to be very precise this nomenclature is not needed to get to the right answer .

Yes you are correct with the usage of which and that.
But here "where" is used for number agreement.

What is subject here ?? a Decision or numerous licensing issues ??
I felt subject is decision and so it is consistent with verb "was"

Change the order:
The numerous licensing issues that plagued..... were crucial in the decision to sell the company, a decision...

So we can see that the subject is plural.

numerous licensing issues were crucial in the decision to sell the company.
A/B/C/E all make the same mistake.
just by identifying the subject verb, I could easily get to the answer choice.
